Ceftobiprole medocaril, a water-soluble prodrug converted by plasma esterases to ceftobiprole (SML3638), is a 5th-generation cephalosporin with broad-spectrum efficacy against both Gram-positive bacteria, such as MRSA, penicillin-resistant Streptococcus pneumoniae, and Enterococcus faecalis, and Gram-negative bacteria, including Pseudomonas aeruginosa and a majority of enterobacteria. Ceftobiprole mechanism of action involves inhibiting bacterial growth by binding to penicillin-binding proteins, disrupting cell wall synthesis. While ceftobiprole medocaril is effective against certain anaerobes like Clostridium spp. and Fusobacterium spp., it lacks activity against microorganisms responsible for atypical pneumonia and is ineffective against bacteria that express certain carbapenemases or β-lactamases, including metallo-carbapenemases and extended-spectrum beta-lactamases (ESBLs).
