Claudin 18 (Cldn-18), a gastric epithelium-associated claudin, belongs to a large family of transmembrane proteins that are important components of tight junction strands. Cldn-18 normally forms a paracellular barrier against H+ induced inflammation in the stomach. Cldn-18 is a negative regulator of bone resorption and osteoclast differentiation. Claudin-18, an early tumor marker, expression is elevated in several precancers and cancers including pancreatic intraepithelial neoplasias (PanINs), intraductal papillary mucinous neoplasms (IPMNs), mucinous cystic neoplasms (MCNs), and pancreatic ductal adenocarcinomas (PDACs). CLDN18 is involved in intestinal-type epithelial differentiation in the progression of intraductal papillary mucinous neoplasm.
