General description

骨钙素属于骨钙素/基质Gla蛋白家族。占骨蛋白总量的1~2%。它由成骨细胞制成,是非胶原骨基质的主要成分。它对钙具有高亲和力,并与磷灰石牢固结合。60%至90%的从头合成的骨钙素被掺入骨基质中,在基质矿化过程中它与羟基磷灰石结合。骨钙素的其余部分释放到循环中,可以作为骨形成的敏感标志物进行测量。血清中发现的骨钙素几乎完全来自骨形成,而吸收过程几乎没有贡献。主要作用于骨吸收的药物,例如双膦酸盐,会引起特定骨吸收标志物的快速下降,而骨钙素水平仅在延长治疗期后才会发生变化,这与骨形成标志物的预期动力学一致。在以骨转换增加为特征的疾病(例如骨质疏松症症、甲状旁腺功能亢进症症和Paget′s病)中,血清骨钙素水平升高;在与骨转换减少相关的疾病(例如甲状旁腺功能减退症症和生长激素缺乏症症)中,血清骨钙素水平降低。
